Learning Disability - Care Manager

We are currently recruiting for a Learning Disabilities Care Manager to support service users leaving school and transitioning into adult services.

Your duties and responsibilities will be (but are not limited to)

*Working with the Adult Services eligibility criteria.
*Giving support to carers to help manage service users expectations during these transitions.
*Seek and ensure services are provided, as is suitably possible in the community.
*Elements of care management within the Learning Disabilities team.
*To manage the accurate data and information in relation to the service.
*To contribute to the development and maintenance of high quality service standards in relation to care and protection planning.
*To assess and to deliver services to meet needs of service users with complex issues.
*Decision making for the service users.
*Contribute to the continuous professional development of other staff within the sector.

To be considered for the role, you must have:

*Diploma in Social Work, Occupational Therapy or Nursing.
*Post-qualifying experience (2 years)
*Knowledge of the Adult services eligibility criteria.
*Ability to work on own and as part of a team contributing to identified team goals.
*Ability to work to deadlines.
